Requirements for Healthcare Professionals


1. Full name, home address, email address and telephone numbers
2. Accurate, complete and up-to-date curriculum vitae
3. Two written references from a current and recent employer or colleague
4. Confirmation that the consultant holds Medical Defence Union or Medical Protection Society Indemnity cover and to provide a Copy of the certificate of cover
5. Evidence of a criminal records bureau check
6. Proof of identification in the form of a copy of Passport
7. A copy of visa/work permits if required
8. Confirmation of the consultant’s General Medical Council registration and registration on the relevant specialty register
9. Evidence of Occupational Health status to the Trust’s requirements
10. A copy of the Consultant’s latest Annual Appraisal Form

4. Details of Work Experience
5. Details of Education & Qualifications
6. Details of Full Driving License
7. Disclosures to include any criminal convictions, appearing on Child Protection List and/or Vulnerable Adults and Disclosure of sickness record
8. Two written professional references, one of which must be from current employer or Medical Director
9. Evidence of a Criminal Records Bureau Enhanced Disclosure
10. Evidence of Occupational Health status
11. Evidence of certified current annual training in Infection Control, Resuscitation, Fire & Safety and Moving & Handling
